Equation relating M, m, d, and molar mass (mm)

How do I write an equation that connects the following variables: molarity (M), molality (m), density of solution (d), and molar mass of solute (mm)?

The most important thing are all the steps involved in arriving at the final equation. Thanks for your *delete me*

Well, you can start with the definitions of each, and then try to see what they have in common.

I'm amazed this isn't well-documented to be honest! In fact a Wikipedia search will probably get you your answer, bring it back here when you find it! :)
