What should the title of my data table be?

Hello! I did an experiment that involved converting solid sodium hydroxide to aqueous sodium hydroxide. In my lab report, I have to put my data in a table and give the table a title.

So the reaction is:
naoh(s) --> naoh(aq)

I don't really understand what kind of reaction this is so i dont know what to name my table.
An example of a data table title would be "The synthesis of indigo"

What are the axes of your graph?
